employment objective - title or type of job you would like to get

equivalent - something that has the same value as something else

given names - your legal first and middle names

maiden name - the last name you had before you took on a married last

mailing address - address where you receive your mail
marital status - if you are married, divorced, separated, widowed or

next of kin - a close relative who can be reached to support you
