Then you must have filled out a form or application sometime in your life.

Applications and forms are a part of everyday life.

You need applications in order to get a passport, credit card, job, or a loan.

You also need to fill them out when applying for funding, government benefits/taxes, education, renting or buying a home.

Forms and applications must be filled out properly by answering all questions and printing in neat legible block letters.

For example if you handwrite instead of printing the person reviewing the application may not be able to read it and may reject or return your application. Also if you leave out some information they may not be able to contact you for an interview.

It is an important skill to be able to fill out applications clearly and properly.

You need to understand the words and terms on the forms to know exactly what to do.

Common Words and Terms on Forms/Applications

acceptable - agreeing with something
accompany - to go along with
applicant - name of person who is applying
application - form for applying for work, school, funding, etc.
authorized - allowed
available - free and ready
benefits - advantages
birth date - the day, month, and year you were born
certify - declare to be true
citizenship - being a full resident in a certain country
completed - finished
confidential - private and personal
considered - thought about
correspondence - letter, email or other written communication
criminal offence - charged for breaking the law
date - write in the day, month, year
dependent - someone who is supported or looked after by another (ex. children)
determine - find out or decide on something
disability - not able to do something physically or mentally (deaf, blind, injury, etc.)
document - an official form such as a birth certificate
effective - working and in use
eligible - qualified
employer - the person or business providing the job
employment record - your history of jobs
employment objective - title or type of job you would like to get
entitled - have a right to
equivalent - something that has the same value as something else
experience - details of your work, skills or volunteer history
false - not true
family Name - your legal last name (surname)
fee - amount of money or cost
for office use only - do not use or write in this space
given names - your legal first and middle names
hobbies - favourite pastimes (ex. listening to music, collecting things, sports, etc.)
identification - documents or cards that prove who you are
income - money that you get from your job and other sources
indicate - to show
initial - the first letter of a name
interview - meeting with a person or group in order to get hired for a job
job title - the name given to the job position
landed immigrant - a person who moves into a country and wants to become a citizen
legal name - your name as it appears on your birth certificate (no nicknames or reversing the middle name with the first)
legibly - clearly
maiden name - the last name you had before you took on a married last name
mailing address - address where you receive your mail
marital status - if you are married, divorced, separated, widowed or single
minimum - the smallest amount
minimum salary requirement - lowest amount of money you want to be paid
next of kin - a close relative who can be reached to support you
official - approved as legal
original - the first one
per annum - for each year
permanent - long-term
personal data - information about you (your name, address, etc.)
photocopy - a copy of a paper made by a copy machine
place of birth - the city or town where you were born
position - place or job
present address - your home address where you live right now
previous - before this time
previous employment - jobs that you had in the past
printing - using block letters not handwriting (cursive)
provided - given
qualifications - abilities, skills, and experience
references - people who are willing to give information and support for you
rejected - not accepted
required - needed
residence - home
resume - your written history of work, education, volunteer, achievement, etc.
reverse - on the back side of the form
review - look at or check over
revoked - taken away
salary - amount of money paid for a job
sex - lets them know if you are a male or a female
signature - writing your name
signing - handwriting your name
social insurance number - government number people use for employment or benefits
specified - point out or name
status - position or situation
suitability - right for something
surname - your legal last name
temporary - short term or limited
type - the kind or category
valid - legal and ready to use
witness - a person who observes an event or can swear something is true
